 Immunization requirements for 2017-2018 School Year: All students must show proof of immunizations before attending school. Failure to meet Colorado Immunization Law within 14 days can result in exclusion (suspension) from school until there is compliance with the law. New Colorado Law: Medical and Personal exemption forms must be signed every year by the parent or guardian. We have form copies in the health clinic.

K-5th grade: DTap (5 doses)  Polio (4 doses)  MMR and Varicella (2 doses each)  Hepatitis B (3 doses)

6-12th grade:  Dtap (4 doses), 1 Tdap (The State requires that students entering the 6th grade receive a Dtap booster immunization.)  Polio (3)  MMR (2)  Hepatitis B (3) Varicella (2)

Please send or have the physician office fax (303-326-1227) an updated immunization record to the school nurse.

 Student Accident Insurance: As low as $19 for entire school year!

The Aurora Public School District does not have insurance to protect your child against illness or against accidents that may occur going to and from school, playing on the playground, or participating in school activities, including sports. Student Accident Insurance is designed to provide only accident coverage to students. There are no income requirements. The carrier is K&K Insurance Group, Inc., which offers accident coverage starting as low as $19 for the entire school year. www.studentinsurance-kk.com  and click "Enroll Now" button for more information on student accident insurance. Enter Aurora Public Schools and click on Murphy Creek. Prices range from $19 for accident during school hours only without dental to $107 for 24 hour coverage with dental. 

Insurance coverage for the year begins on the first day of school, or on the date the application is received if later. In the case of school athletic/activities, coverage will begin on the first official day of practice, contingent on premium payment. The expiration date of coverage for those purchasing School-Time coverage is the last day of the regular school term. The 24-Hour coverage provides protection from the effective date to the date before the first day of the next school year. Aurora Public School offices will keep a supply of enrollment and claim forms.
